Hombres Health ( “Hombres Health” or “we” ) owns and operates the websites located at www.hombres.health (the “ Website ”) and Hombre Health is owned and operated in the past, present and future (collectively, the “ Platform ”). All patient information and data that is defined as “protected health information” (PHI) by HIPAA statutes will be kept confidential in accordance with patient privacy law statutes.
By accessing or using our Services, you agree that although HIPAA applies to Hombres Health, Medical Groups, Providers or Pharmacies, any information you submit to Hombres Health is not intended and used solely for the provision of diagnosis and treatment by the Medical Group and the suppliers or the fulfillment of the prescription by the Pharmacies, is not considered Protected Information.

When you use our Service, we collect all information that you provide to us, including, but not limited to:
